• Use the CoWIN portal, Aarogya Setu app or log on to https://selfregistration.cowin.gov.in/

• Enter your mobile number

• Get an OTP to create your account

• Fill in your name, age, gender and upload an identity document

• Choose K.J. Somaiya Hospital after entering the pin code 400022 and your preferred date

• Up to 4 appointments can be made by one mobile number

• The cost per dose of Covishield is Rs 780/-
